Facility Information: Square footage: 6,694. Facility located in a large, two-story, renovated, brick house. This site also has a two story annex that is used for meetings, medical exams, training and administrative offices. The center is handicap accessible with ample parking. The site is zoned "neighboring office" and is surrounded by single-family homes and an apartment complex. It has easy access to interstate highways and public transportation.
